Good morning! Several students have tried to complete the participation form sent to you, yesterday.
Please note this additional information which is needed to complete the form:

Page 1 of the form asks for trip details. The attached form has page 1 completed. You may use the attached form and complete it as per the instructions below or use it to obtain the info needed for page 1.

When renaming the form in the required format, use TIME 2000 as the course section and SPR 23 (not FA22). See the example here:
Required: First Name Last Name_(CUNY ID #)_Waiver_Course-Section_FA22
For example: Joe Smith_(11111111)_Waiver_TIME 2000_SPR23

Dear TIME 2000 Students,
Happy new year! I hope you are enjoying the winter break. If you are attending
LIMAÇON
in March, the college requires that each student complete and submit a
Participation, Waiver, and Emergency Contact Form. Please read the instructions below and then complete the form. Note that it must be sent to two people, Sadia Ishak in the Provost’s office and me. The deadline for submitting the form is February 8,
2023, but early submission would be appreciated.

Complete the form and save it. See instructions below regarding how to sign the form electronically. If you have difficulty with the e-signature, you can print the document, sign it, and then send a scan or photo of the form. You may also email Tobi for
help with the e-signature at

Rename the saved document using the following format with your name and id:
First Name Last Name_(CUNY ID #)_Waiver_Course-Section_FA22

How to electronically fill out the permission slip-
For Microsoft Edge and Firefox Users:
1) Open the contact form link:
https://www.cuny.edu/wp-content/uploads/sites/4/page-assets/about/administration/offices/ehsrm/risk/travel/FinalOffCampusActivityParticipationWaiverandEmergencyContactForm021512.pdf
2) In the PDF viewer, fill out your emergency contact information using the fillable text boxes on page 3.
3) Insert your signature on page 3 by using the textbox tool or drawing tool.

4) Save the form, and email it to TIME 2000.

1) Download the contact form:

4) Insert your signature on page 3 by using the textbox tool or drawing tool. These can be found in the area I circled.
Place your signature on the signature line.
5) Download the form using the button in the top-right corner, and email the form to TIME 2000.
