• Staff members please complete pages 1 & 3, then return to your area supervisor for review and approval. 

• Directors who report to Jenn, please complete pages 1 & 3 then, return to irving.uribe@qc.cuny.edu by Tues., 7/6 for Jenn’s review and approval.

• Directors please complete the “proposed Fall 2021 Schedule for Employees” excel document for your department then return to irving.uribe@qc.cuny.edu by Tues., 7/6 for Jenn’s review and approval.
