Hi Club Members!

The name question did not make it on the google form for the shirt/last club day registration. The problem is fixed, so if you have already filled out the form please fill it out again. I'm sorry for the confusion! Here is all the registration information

This registration form is to help us get an idea of how many shirts we will need to order. You must attend the last club day to receive a shirt. Please fill out this registration form by November 1st. 
Shirts take about 1-2 months to receive so make sure you are using a permanent address on the form. We are also trying to get long sleeve shirts for the fall if it fits in the budget so pick your size accordingly.

  1. If you will be attending the last meeting
  2. Your shirt size
  3. Address (please make sure this is correct)
Don't forget! T-shirt designs are due by November 1st 🎨
here are the t-shirt design rules 
  1. Logo must include QC Dietetics or QCDC
  2. Logo design must be a maximum of 3 colors. Keep in mind the logo will be on either a white or black shirt (logo designer can pick the shirt color) 
  3. Logo design needs to be emailed to qcdieteticsclub@gmail.com 
After all, designs are submitted a google form will be sent out to all club members to vote on the design they like most.
