If an error has been made on a student's attendance record, you must complete the Commencement of Attendance Revision Form (formerly the Attendance Revision Form) and submit it to my attention.

You can access the form online at http://qc.cuny.edu/?id=TE61. You will need to log in to MyQC using your CAMS ID (not your CUNYfirst ID). You can also find it on the MyQC homepage, under MyQC links on the right-hand side.
