Here you go, Betsy. The mentoring manual -- which the observation form is keyed to -- is so detailed that it may be more useful to have the "interaction guidelines," a one-pager sent out to all faculty
by their academic directors (our equivalent of dept chairs) at the start of each term.
I also have appended not just the standard contract but the request form that must be filed by the AD and approved before hand, as well as the approval sign-off. That's the full arc of the process.
There are also something like seven variants on that contract (for minor revisions, for multiple authors, etc.).
